I hereby authorize a sponsorship commitment to United Way of Greater Milwaukee & Waukesha County for the following opportunity.

• Logo as sponsor on all event materials, programs, screens/monitors and signage. 
• Complimentary attendance for 5, with special sponsor nametags.
• Recognition in United Way publications and on United Way’s website. 
• Inclusion in the 2025 Closing Celebration program reaching more than 400 community business leaders at the December event.
• Company inclusion, with logo, on the Corporate Sponsorship area on United Way’s website.
• Electronic billboard recognition approximately 300-600 views per week on 40+ locations. 
• Other special recognition opportunities if they become available.
