Funded Programs
Clinical Services: The Drop-In Center provides a non-traditional model of trauma-informed mental health intervention focused on improving immediate life stressors by meeting basic needs and supporting youth in goal achievement via ongoing supportive services. (Health)
Community Case Management: In partnership with the Greater Milwaukee Foundation and in alignment with the Family Flex Fund, this grant will provide flexible financial assistance for community case managers. (Safe & Stable Homes)
Pathfinders Youth Shelter: Youth engaging with the Shelter have access to comprehensive youth and family counseling, case management, and supportive services that include basic needs assistance, aftercare, street-based outreach to at-risk youth, educational workshops and groups, youth leadership development initiatives, and positive peer activities that provide safe and structured opportunities for growth. Preventing Family Homelessness for Young Adults: Scattered-site transitional housing model that offers person-centered wraparound supportive services for youth-led households (ages 16-25) that invest in safety, positive connections, education and employment, life skills, and social-emotional wellness. Prevention Education Program: Incorporates prevention education around teen pregnancy, STIs, and sexual violence to empower youth who attend middle schools and youth-serving community-based programs, with expertise in serving runaway/homeless youth populations. (Health)
Supported Housing: Utilizing a Housing First model informed by positive youth development, harm reduction, and trauma-informed care practices, all programs under Pathfinders' supported housing umbrella offer safe, stable, and supported housing “first" for youth and young adults ages 16-25.
