It's important to prioritize good health at every stage of life, from conception through childhood, into adulthood, and through older age. A community's well-being is closely tied to the health of its individuals. But too many people struggle to afford the care they need.
Recognizing the need for equitable access to healthcare, United Way of Greater Milwaukee & Waukesha County and the Milwaukee Health Care Partnership (MHCP) established the Health Improvement Fund. This initiative aims to provide support to not-for-profit health and human service providers in our community, ensuring that everyone has an equal opportunity to be healthy.
In 2024-25, we are funding the work of five programs dedicated to improving health outcomes for low-income and vulnerable individuals in our community.
Meet the 2024-25 grantees for funding:
Advocate Aurora Walker’s Point Community Clinic
Program Purpose: Primary Care for Uninsured
Program Description: This program provides primary care services for underserved patients in Milwaukee County.
Ascension Wisconsin Foundation
Program Purpose: Angel of Hope Clinic
Program Description: This clinic provides integration of behavioral health and primary care for underserved patients in Milwaukee County.
Ascension Wisconsin Foundation - Seton Dental Clinic
Program Purpose: Dental Care for Uninsured
Program Description: This dental clinic provides free, comprehensive dental care for youth and adults in Milwaukee County.
Bread of Healing, Inc.
Program Purpose: Health Care for the Uninsured
Program Description: This clinic provides primary care services for uninsured patients in Milwaukee County.
Community Smiles Dental
Program Purpose: Healthy Smiles for Milwaukee County Patients
Program Description: This dental program provides free, comprehensive dental care for youth and adults in Milwaukee County.
Beginning in 2024, The Mental Health Improvement Fund (formerly the Health Improvement Fund) will support organizations that provide mental health services for low-income, underserved adult and youth populations in Milwaukee County. The goal is to increase access to mental health care, improving mental health outcomes, and promoting health equity.
Grants are awarded to organizations through the Milwaukee Health Care Partnership (Ascension Wisconsin, Advocate Aurora Health, Children’s Wisconsin, Froedtert Health) and United Way.
Supported activities include increasing access to mental health outpatient diagnostic, treatment services, and improving care coordination. Grants are one-time investments to nonprofit, health and human service agencies in Milwaukee County.
